ABMS PORTFOLIO PROGRAM 

ABMS PORTFOLIO PROGRAM SPONSOR - MOC Certification

BACKGROUND

Convergence Health is now a Portfolio Program Sponsor with the American Board of Medical Specialties Portfolio Program™ (ABMS Portfolio Program). Through the ABMS Portfolio Program, we can help physicians and physician assistants earn continuing certification (MOC) credit for their QI work from participating ABMS Member Boards or from the National Commission on Certification of Physician Assistants.  

GETTING CREDIT

To participate, you must be enrolled in your certifying board(s) MOC program. To receive credit, you’ll be asked to complete an attestation of your work. Convergence Health will take care of the essential administrative tasks related to credit confirmation. 

Currently, Convergence Health provides continuing certification credit for the following programs:

To receive credit, please complete and submit this attestation form by December 1 of each year. Once submitted, the completed and signed attestation will be retained by Convergence Health. We will then notify the Portfolio Program of your completion. You will receive a copy of your attestation via email for your records.

  • American Board of Obstetrics and Gynecology 

  • American Board of Ophthalmology 

  • American Board of Otolaryngology – Head and Neck Surgery 

  • American Board of Pathology 

  • American Board of Pediatrics 

  • American Board of Physical Medicine and Rehabilitation 

  • American Board of Psychiatry and Neurology 

  • American Board of Radiology 

  • American Board of Surgery 

  • American Board of Thoracic Surgery 

  • American Board of Urology 

  • National Commission on Certification of Physician Assistants
